Enrollment Requirements

Along with the required fees due at time of enrollment, the following forms must be filled out:

• Signed Tuition Agreement
• Enrollment Form
• Personal History Form
• Health Statement with Immunization Record
• Emergency Release Form
• Child Release Policy
• Acceptance of "FMA Student Policy Manual"

A copy of your child’s birth certificate will also be needed for our files.
